Automatic Title on Form

    I have a title posting on my form that reads "My Form Post" after embedding it in my site. I'm not sure how to get this off. I've titled my form but this still shows up. You can see this by going to http://ineedablackhairstylist.com/?page_id=746

    I've figure it out. My programmer added it in. It wasn't a Gravity Forms issue.
