It does not appear that the Gravity Forms forms.css stylesheet is being loaded. Did you insert that sidebar form using the Form widget or a PHP widget or some other way? You should have the Gravity Forms stylesheet loaded to get rid of the bullets and apply other styles to the form when a validation error occurs.
If you look at your Contact Us page where there is another Gravity Form http://new.seoclarity.net/contact-us/ you can see there are no bullets and there is no border around the button.
If you are intentionally not including the Gravity Forms CSS, please let us know that and we will work around it, but including the CSS by using the Form widget is the easiest way to style this sidebar form.
