Bug in GravityForms

    Hello,

    I have a form at the URL : http://www.gargiz.com/store/create-short-video/.

    On page 2 of this form, the form has
    1. Additional Customization Options - A multi-select checkbox option field - With 6 options listed
    2. Your audio clip - A File upload
    3. Your English text for adding audio - A paragraph text box.

    However in the preview and also on the page described above, the second page only displays
    1. Additional Customization Options - without any options listed

    I need this corrected ASAP. Can you help?

    Please export this form as XML (Forms menu > Import/Export sub menu, then top center menu item "Export Forms") and email it to me at chris@rocketgenius.com and I will install it and check the configuration. Thank you.
