Calendar Icon NoT Displaying

    /////// EDIT I had to manually add 2 scripts to the HEAD of my theme. It's working now.

    Hello,
    I have searched through some previous posts and have not been able to find a solution to my problem..

    I have a form that requires a user to select a date on my website page:
    http://www.iheartlongisland.com/submit-long-island-photography-snapshot/

    On the main site the calendar icon shows up no problems... however I created a stripped down version of the site so that I can run it on Facebook in a iFrame and the calendar Icon is not showing up as seen here:

    I see that it has the reference for the calendar icon in the HTML but it is not displaying.

    any help would be appreciated. Thanks,
