Can Gravity forms do something like this?

    My client wants a poll similar to this, with results databased for future use.

    Does Gravity Forms support groups of labels with multiple choice options horizontally for those labels on the poll add on?

    Sounds like you are using the Poll Add-on already?

    If so, can you post an example of your form online now? We can usually style the form to lay out however you like with CSS.
