Cart66 checkout showing user input form?

I am using Gravity forms with cart66 to create a number of product options. After the user makes product selections, the cart page is showing a user input form with a button that says "save" along with a link which says "Show Details."

There is also a strange looking ":" above the box.

I would like to remove all of this so that only the product and selected option show.

The store page is at http://www.focaluprightfurniture.com/store (not ready for full purchases yet). Add an option for the seat to see what I mean.

The desk below the seat is not using Gravity forms and shows in the shopping cart properly. I'd like the seat to show the same way.

After many hours of trying to figure this out, I finally found the problem - a setting in Cart66. Looks like all I need to do to fix these things is post to this forum.
