Changing the form title only on the front end

Is there a way to change the form title on the front end of the website?

ie: if your form on the back end is called "Contact Us" and you want to display it on a particular page as "Get ahold of us".

    Thanks!

  2. Sure Nick, you could opt to not display the form title when you embed or place the form on the page, and then just put whatever title you want above it in HTML as part of the page/post.
