Check boxes lose their values on submit

    I have a form that is used to collect a referral for our service. The first question on the form is a checkbox group with 5 possible selections. No matter what selections I make, when I submit the form I am presented with an error (this is a required field) and all the selections I made are unchecked. If I re-check them and re-submit, the form works fine.

    There are other forms on this site that work appropriately.

    Hi,

    I just submitted the form without experiencing any issues with the checkbox group using chrome 27, which browser and version are you using and have you tested it in any other browsers?

    Hi Richard and thanks for looking into this. I am using Chrome on Windows 8/x64. I had not tried it in Firefox or IE so I'll do that. Thanks for the suggestion.

  4. Bob, are you using auto-fill in Chrome? There is a known bug in v.27 that will be getting corrected in v.28 where when using auto-fill radio and checkbox items that were selected prior to using autofill clear the selection(s).
