Checkbox field label not showing in entries table

  1. Hi,
I am creating a questionnaire with >50 questions for the upcoming election. About half of the fields are Yes/No type questions and I'm using checkbox fields for them. When I view the entries the checkbox field labels are missing in the header of the entries table and instead I see "Yes" and "No" over and over. This is not at all helpful, as I need to see the question, not just the response. I must be doing something wrong but don't know what. Here's the form so far: http://www.boulderblueline.org/test/ use 4880 for the code.

    Thanks!

  2. Do you mean on the entry summary page for a form, or the actual view of one entry? Can you post a screenshot of what you're looking at specifically? Thank you.

    This page:

Or when you click on any one entry from that page?
