Hey Guys, couple things here.
Possible bug?? I updated to the latest gravity forms today and now my conditionals aren't working. Spent HOURS building a very intense form originally and now I have to start from scratch all over again.
The other problem I'm having is all the forms are being sent out with MY email address on them instead of the email address I specified in the "From" section
Can you elaborate on your conditionals not working? What exactly isn't working? Does your form appear but the fields don't show/hide? Does your form simply not appear on the page? Does the form appear correct when your Preview it in the admin?
If the email notifications are being sent out with a different Send From than what you specified in your notification settings then this is most likely a theme or plugin conflict caused by your theme or another plugin filtering the wp_mail() function and hardcoding the send from.
To see if a theme or plugin is causing this issue, and it could be causing your conditional issue also, you must follow the instructions here:

Everything from "Session 1" down should show hidden choices and add or subtract fees from the bottom. The form is not working correctly in preview either. Sessions 1,2 & 3 should allow the user a drop down file and check boxes as well as add prices to the bottom with a discount for multiple sessions. Choosing "on Campus" should open up more choices (ie. private room, roommate etc) as well as add cost to the bottom figures.

One question regarding your conditional logic, are you using a lot of Section Break fields and applying conditional logic to Section Breaks?
If so, that can cause issues. When you apply conditional logic to a Section Break it carries down to ALL fields that appear below that Section Break up to the next Section Break. So it can cause fields to disappear or show that you may not have intended because of how you have used Section Breaks.
Can't see the form so dunno if thats going on, but it's a common issue to new users when using conditional logic and section breaks.
server hiccup, it's working again now. Not sure what "Section Break" means. I had everything working before the update. *sigh*
