I have multiple forms on our site, many of which use conditional logic. In some instances, fields that use conditional logic will not show, others will. Here is a link to an example of two forms.

The first radio button titled "select a ministry" should have additional choices pop down below for each different radio button selected. All are entered correctly, however only a few work.

On this form, when you choose a location, another conditional logic field is supposed to display below asking them to choose the time they attended based on which location. This conditional logic does not display at all.
