Conditional Formatting breaking forms

    Hi Guys,

    I can't for the life of me work out what is happening, but any forms with conditional formatting are not being displayed on the site.

    Forms with out conditional formatting are displaying as normal

    All fixed up... There was a jQuery error that was causing the issue.
