Conditional Forms Not Displaying

    My contact form with conditional fields seems to have stopped working during the past week.

    http://www.heyitsfree.net/contact/

    If I switch W3 Total Cache off, the form loads fine. Furthermore, if I disable JS minfying, the form loads fine. I only have three JS files in the minify settings:

    Any idea what I'm doing wrong here?

    jQuery isn't being loaded on the page, so that's the main issue. Are the file locations correct for w3 total cache's minify settings? I'm not too familiar with the minify settings there.

    As @kyle said, the necessary jQuery isn't there for the functionality to work. You may want to look into your W3 Total Cache settings and see what is going on. If it works disabling JS minification you may need to leave it disabled.

    The middle file was an old file. *facepalm*
