Conditional Logic Form Not Displaying on Post Page

    My form should appear on this page between "hello" and "world":
    http://www.careergreenlight.com/contact/

    But I do not see it. When I view source, I do see it there. What am I doing wrong? Thanks!

    Another script on your page is breaking the Gravity Forms conditional logic script so the form isn't being displayed properly. When inspecting the page, I see a script error related to the "sharebar" plugin.

    Apparently the plugin is outputting script on pages where it shouldn't and causing problems. Other users have run into the same issue and when they disabled the sharebar plugin, their forms worked properly.
