Conditional logic issue

  1. Hi,
    I'm having problems with conditional logic on some of the fields on this form: http://sptc.the-graphics.net/about-sptc/membership/application-form/
    In the 'Membership' section, I have a field called 'Ordinary membership (PC)' which should only be visible if:
    'Type of organisation' is 'Parent Council' and
    'Please select the type of membership you want' is 'Ordinary membership'
    ... but it appears as soon as I set 'Type of organisation'.
    (There are two more 'Ordinary membership' fields for other types of organisation which are behaving in a similar way.)

    Any thoughts on how to get this working properly?

    Can you double check to make sure you have your conditional logic set to ALL of the following and not ANY of the following? As that is usually what causes such issues.
