Conditional Logic Works on half of my fields

    Hello, I am using the current version of GF 1.6.12
    Also using Stripe and Mailchimp integration.

    Here's my page:

    I have conditional logic on the price radio buttons.
    When 3man team, or any level of sponsor is clicked, -> Name #2 Address #2 Phone #2 Email #2 and Handicap #2 Name #3 Address #3 Phone #3 Email #3 and Handicap #3 is supposed to appear.

    When clicking a sponsor level, the sponsor information is also supposed to appear. The sponsor info appears as it should, but why doesn't the other information follow the conditional logic?

    I figured out that I was using one form element to control two different sets of conditionals. Once I separated the controlling element into two parts, things worked as expected.
