conditional shortcode in email notification

    I am getting the error message:
    Oops! We could not locate your form.
    Oops! We could not locate your form.

    Here's my conditional:
    I hereby give consent that the child(ren) listed may travel with their
    [gravityforms action="conditional" merge_tag="{Who is travelling with the child(ren)?:19}" condition="is" value="Father"]Father[/gravityforms]
    [gravityforms action="conditional" merge_tag="{Who is travelling with the child(ren)?:19}" condition="is" value="Mother"]Mother[/gravityforms]

    The rest of the email displays normally.

    Can you link to your form please? Are you using the latest versions of Gravity Forms and WordPress? Do you have any other conditional shortcodes in your notification?

    Hi David, I have no other conditionals, just regular code. Here's the form input page: http://gwdev.business-sitting.com/resources/form/
    Using WP 3.5.1. and only just purchased Gravity, so the latest version.

    Fixed the issue - seems there were extra "codes" put around the shortcode when pasting into the visual editor. Removed them in the text editor.
