Confirmation redirect not working

    My email submission page at http://www.elementsuser.com just stopped working properly. Visitors are supposed to be able to enter their email address, click the submit (aka "Download Now") button, and be redirected to this page: http://www.photoshopelementsuser.com/elementsuser-starter-kit-1/

    This was working fine until this morning.

    Now, the form still logs their email address, but fails to send them to the confirmation page. I've tried using the "Page" feature and putting in a URL directly.

    I'm at a loss as to what might have happened.

    I saw a couple problems on that page. There is a double AJAX spinner after submit, and also this JavaScript error after submission:
