Okay, found it. I was finally able to recreate the issue when previewing a form with conditional logic. The GD Star Rating plugin is outputting javascript on admin pages it doesn't need to and it's breaking the conditional logic script.

It's pretty common for developers to globally output code and this is the cause of most of the plugin conflicts we see.
This isn't a Gravity Forms issue, but one with the GD Star Rating plugin. You can try to contact the developer of that plugin and explain the issue. If they simply restrict their script output to their plugin admin pages, there shouldn't be any more conflicts.
