Contact form per product/post basis, form must display the image.

I need to create a WordPress based catalog/CMS. At the moment in lieu of a cart we need to have contact form accessible on a per product/post basis. The form must display the product image, product image, SKU plus other dynamically completed form fields. Lastly, we need to store the message in a CRM app like Salesforce, or anther CRM app so we can track/manage the conversation.
