Your forms look great. Having design templates I could plugin to your forms would be the icing on the cake.

There aren't any design templates included, just the default styles. We may add some kind of a theme pack add-on in the future though.
The forms are intentionally generic and are intended to actually inherit some of the styles from your theme. Our thoughts are that the theme design should really drive the look and feel of the forms.
