Duplicate Form losses CSS

    When I duplicate a form I loose some of the styling from the original. Seems like mostly list styles. Anyone else have this issue and know of a fix?

    Can you embed the two forms to pages and link to those pages so we can see the differences between them to better understand what might be happening?

    After some investigating, it looks like it has different li#field id's

    Any help is appreciated.
