Duplicated form appearing on page

Good afternoon.
I tried searching for posts about this but couldn't find any and was looking for some help.

After creating a form I dropped the code in (on the HTML editor), but for some reason, the form is duplicated on the page. In addition, it looks as if there are two different styles to the forms and the second form (which I don't understand how or why it is appearing and don't want) also has a Captcha, which I didn't add. The only coding at all on the page is the Gravity Form.

I tried toggling the No-Conflict mode in the Gravity Form settings, but that didn't seem to resolve it.
