Is it possible to send an email notification of the entire form, all it questions and answers, even the ones that are not checked off.
We are doing an online medical evaluation, and require to see the entire results, even if they did not check the box off?

The actually patient history is a couple page breaks into the form, but you can see the questions and the check box to mark the symptoms.
