When I try to export the "Waiting icon" just keeps turning round, but nothing gets exported. I don't get any error message.

Strange, the plugin does create the folder. We haven't run into this issue in our testing. It is probably a server configuration of some sort. It's tough to debug these things with so many different possible server configurations, etc. Glad you got it working.

what folders do we have to create manually? Where? I am having the same issue...

Gravity Forms creates a "gravity_forms" folder under your WP uploads folder (usually /wp-content/uploads). Make sure that folder and all its subfolders are writable (permission 755). I manually created the folder at /wp-content/uploads/gravity_forms and set permissions to 755, and when that didn't work, I tried 777. Are additional subfolders needed?
