Form disappears on a couple of pages

    Today, just like some other people I saw on the forum, I updated the plugin (like I always do, as soon as there is one)
    Now the form disappears on a couple of pages;

    Here are the two pages where the form disappears:

    - http://www.suurdongediertebestrijding.nl/contact/

    And here it's still visible:
    http://www.suurdongediertebestrijding.nl/plan-bestrijding-in/

    There been no changes recently.

    Looks to me like it is because you have your theme set-up to call certain things in the footer, and as such, the conditional logic isn't working correctly, so when you have a form with conditional logic, it puts the JS for that in the footer, and as such the form is never revealed. You'll want to move this to your header.php file or contact your theme developer to move these calls to the header in order to make those forms work correctly.
