form does not appear on the page

    I created a form and it does not appear on the page, which may be the reason.
    Ja did everything. redid 2 times more when I send the form to insert it does not appear on the page. What should I do. it appears in the pre-vivisualisação.

    I found that the problem is on the theme, thank you.

    Crashed in with the theme. (Wellness) DOES NOT WORK MORE
    With the code "[gravityform id =" 20 "name =" Test Form "]"

    Help me understand. (Uninstalled and installed everything new in this field "www.simcontatos.com.br)

    I created forms for a site with version 1.6.9 of GRAVITY FORMS
    Today I created forms for the same site with the version of GRAVITY FORMS 01/06/11

    I put the Forms created (GRAVITY FORMS 1.6.9) on the page: http://simcontatos.com.br/accordion/
    (OK)
    And also created the form (GRAVITY FORMS 1.6.11) on the same page.
    All the forms created starting from version 1.6.11 no longer appear.

    Oops! We were unable to find your form.

    Only problem might be in version 1.6.11

    When I create "section break" to "Enable Conditional Logic"
    The form does not appear.
    When I remove the "section break" the form appears.

    What should I do to fix this and be able to use "section break"

    Does Gravity Forms work currently with the Twenty Ten or Twenty Eleven theme? You mentioned a few posts back that it does. If that is the case, then the issue is with the theme. Let's stick to one Gravity Forms installation on one domain. Please be sure you are using the latest Gravity Forms 1.6.11 and WordPress 3.5 and post the link to a form on one domain which does not display. We will troubleshoot just one of these and hopefully find a common problem affecting all your sites. It seems like most of them I checked were using the Wellness theme from Themeforest?

    The issue occurs when you use conditional logic. It's because you have a JavaScript error on the page. The JavaScript error prevent Gravity Forms from revealing the form properly.

    Working with 6 domains, using the same theme, "Wellness theme from ThemeForest." And all using the Gravity Forms 1.6.11

    The common denominator here is the Themeforest theme. Because the forms work when using the default Twenty Ten theme, the issue needs to be addressed with Themeforest. I looked through the source of the page and can't see any obvious JavaScript or jQuery errors, but I'm not an expert in Themeforest themes. Please bring this issue to their attention. Thank you.
