Form does not display

  1. Total newbie here! Just purchased a license and built my first form using conditional logic. The form works perfectly in preview, but does not display on a page. I have turned off all plugins and the form still does not display. I'm thinking its a JQuery conflict, but do not know how to fix that. Any help would be appreciated. Here's a link to the form:

  2. Hi, the form on that page is showing up fine for me in both Firefox 12.0 and Chrome 19.0.1084.52 m on Windows 7. In what browser are you experiencing the problem?

  3. I got it. It was a JQuery conflict. I wasn't using wp_enqueue_script() ;-)
