[RESOLVED] Form missing all settings

Built a complex form, tested, and everything worked. Today, the form shows up as an entry in WP Admin but when I preview it or try to edit it or view it in a page on the front end, none of the forms settings / fields show up ( "Oops, form not found" on front end, blank form on back end ). Checked the database, the form has all of the meta data still intact.

Any known causes for this sort of behavior ( to narrow our debugging )?

Can you go to Forms -> Settings and turn No Conflict mode on and see if that helps? What version of WordPress and Gravity Forms are you using?

    No conflict already on.

    PHP Version 5.3.3
    MySQL Version 5.0.77
    WordPress Version 3.5.1
    Gravity Forms Version 1.6.12

What has changed recently? Can you try the following? http://www.gravityhelp.com/documentation/page/Testing_for_a_Theme/Plugin_Conflict

After that, if it still doesn't work, can you try the following meta recovery plugin found on this page http://www.gravityhelp.com/downloads/

    David,

Nothing changed about the theme and/or plugins.

I am trying the meta recovery plugin now.

    David,

The Meta Recovery plugin worked. You, sir, are a life saver! Thank you!
