Form Missing from 'Add Form' dropdown

    I have a site that has a few forms on it. When I got into the page that I need to have the form on and click the 'Add Form' button, two of the three forms are missing. I have tried adding the shortcode to the content directly, but the form does not show on the front-end page either.

    Here's the information about the install:

    A few people have experienced issues recently with jQuery 1.9, try downgrading to version 1.8.3 which is the same version wordpress ships with and see if that makes any difference.
