Form not progressing past first page

Hello. I was having issues with my form not submitting even though it was sending according to the client. In an attempt to try and help this I have moved the form over several pages now, it works fine in wordpress but when it's live it won't progress past the first page. when you hit next it kicks back to the first page, any idea why this is happening?
