You can password protect the page where the form is embedded but you cannot protect the form itself with a password. Forms can't be used outside of a WordPress Page or Post.
Is that what you tried and it did not work?
You could also require that the visitor be a logged in user, but that seems like overkill for what should work straight away.
If you can provide a link to the page with the form on it, maybe we can provide more specific advice.
This WordPress Page is password protected (the password is gravity). It should still work fine and not redirect you to the wp-admin. (I just tested it to be sure)
