Form shows in Preview, Disappears when Page Is Published!

  1. Here's an odd error I'm getting with Version 1.5.rc3...

    I have had a "Bug Report" form on my website for a while -- which worked well until recently:

    Not sure when it started, but the link above now generates an invisible form. (Viewing the page's source reveals that the form data is sent, but hidden via a CSS style="display:none" reference at the beginning of the form's HTML).

    Even weirder -- when I am editing the page, the form is visible via the page preview in WP. Hit publish/update and it vanishes! I've tried making a new page and inserting the form = same invisibility issue. I've also tried duplicating the exiting form into a new one and calling that form id = same trouble.

    Any suggestions?

  2. There appears to be a Javascript error related to conditional logic which is why the form isn't displayed. Can you send us a WordPress admin login for this site via our Contact Us form so we can take a look? Thanks.
