I cannot collapse the add new form fields in the admin screen and when I do add a new field, such as name I cannot drag it and move it as per the screen shot.

Please let me know what I need to do to rectify this situation.

Very likely a JavaScript problem. Do you have any themes that add JavaScript to the site admin? Chances are you need to disable your plugins one at a time until you find the culprit. It is possible there is something in your theme causing the issue as well.
Yep, this is usually a theme or plugin conflict. Something is enqueueing other scripts on the Gravity Forms admin pages and causing a conflict with the script used to run the form builder UI. You'll need to try testing for conflicts as described at the page below and see if you can narrow it down.
