Forms Not Appearing

WP 3.0
GravityForms 1.3.12.2
Gravity Forms Widget 0.2
PHP 5.2.11

I created one form and place it in the sidebar using Yoast's widget. The form does not appear.

I also placed the same form in a post using shortcode. The form does not appear but the following error does;

'Oops! We could not locate your form.'

I am assuming it is a PHP error. Does something else need to be activiated?
