Forms Not showing on pages

HI

I added gravity forms using user registration addon but forms are not displaying on front end pages

example page: http://expatsinasia.com/employer-registeration/
form is not showing and when i look using firebug( forms showing with display:none)

I tried all options according to FAQ or other answers in forums*like deactivate all plugins,adding wp_footer in theme etc but no solution worked

I am using striking theme for site

Gravity Forms needs JavaScript to utilize conditional logic. Until that's loaded, the whole form will be hidden with display:none.
