Form's submit button not working

  1. My apologies in advance for possibly posting a dupe. I've searched and found slightly similar posts, but none have a solution.

    The form in question is here: https://www.52novels.com/query-us/

    It's the one in the sidebar. Clicking submit does nothing. I have two other forms---created from duplicating the form above---that work just fine. See them here: https://www.52novels.com/privacy-policy and here: https://www.52novels.com/terms-and-condtitions/

    The broken form stopped working the day I created the forms on my privacy policy and terms pages. The preview of the broken form works. The form is broken in IE and Chrome. It doesn't make sense to me that there'd be a theme or plug in conflict when identical forms---as well as all other forms---still function.
