My apologies in advance for possibly posting a dupe. I've searched and found slightly similar posts, but none have a solution.

The broken form stopped working the day I created the forms on my privacy policy and terms pages. The preview of the broken form works. The form is broken in IE and Chrome. It doesn't make sense to me that there'd be a theme or plug in conflict when identical forms---as well as all other forms---still function.
