Forms with Conditional Logic will not display

    I have a form that I'm trying to place on a page, and after much hair pulling I've found that it will only display using the shortcode when the form has no conditional logic set, but when there are fields with conditional logic it will not display.

    Any suggestions for getting this to work? My form requires that I use conditional logic in it...

    When a form with JavaScript does not display, it is normally a jQuery or JavaScript error. If you post a link, we can help you find that.

    JavaScript and jQuery problems are usually caused by theme or plugin conflicts. You can begin troubleshooting by following these instructions: http://rkt.gs/testing

    Did you start checking for plugin and theme conflicts yet? Looks like there is (at least) a JavaScript error coming from WP E-commerce?

    I still get the same issue even with wp e-commerce completely out of the picture... hmmm

    Have you gone through all the troubleshooting tips? All plugins deactivated and a default theme (Twenty Eleven or Twenty Ten.)
