Carl, I followed your instructions (see below) and found the culprit to be the Google Website Optimizer for WordPress plugin by ContentRobot. When I deactivate it the form displays fine. Do you have any suggestions for a fix? I have to use Website Optimizer but I can use a different plugin that won't conflict. Any ideas?

This could be a number of things, a possible conflict with another plugin, an issue with your WordPress install or an issue with how the page or post was saved.
If you paste a shortcode in when in the visual editor and not the HTML post editor this will cause the shortcode to display. Make sure you are in the HTML edit mode and then insert the shortcode.
If this doesn't work you will need to deactivate ALL your other plugins and then test it and see if it works. If it does, then activate each plugin one by one and test the form after each one to see if it outputs. When you encounter a plugin that causes it to stop working, that plugin is interfering with Wordpress shortchode output.
Shortcode functionality is built in WordPress functionality, so there is something going on with your WordPress install. Try the above options and let me know how it goes.
