Gravity Forms Table Layout

I am in need of making somewhat of a hybrid table/gravity form. I need a 3 column order form (where column 1 and 2 makeup a item number and a description. Column 3 is simply a quantity field.) Quantity fields are filled where needed and the output is emailed while maintaining the same table layout/look (this is for the sake of the customer service people who need to enter the item number and quantity into the company's inventory system.

I would like to maintain a similar look while moving to the gravity forms platform.
