Help! Form has disappeared . style=Display:none on gform wrapper

    Hi,
    I've just taken a look at one of my live sites and my main form has 'disappeared' from view.
    I can see that the last entry on the form was 24 hours ago. The only mod I have consciously made in that period is to add a favicon link in the header.php.
    The form does have conditional fields on it which I know can cause sections to be hidden, but it worked perfectly up until yesterday.

    The form is at the foot of the text on the page titled:-
    Sí, quiero apuntarme ya

    Any thoughts would be gratefully received

    My personal panic over. the js files required for forms were not loading and for that reason my form was set as 'display:none'. Re-instated them in the header and it all works swimmingly again.
