How can I send the Site Title and a Custom field

    Description: I have this site with a lot of events on the right side like this: http://hardthaus.de/feinschmeckertermine/silvester-party-2011-2012/

    All events have already a form to make a reservation in the restaurant.

    But we want to make a button with a link to a site to order this Event as a voucher. I have already a form to order a voucher: http://hardthaus.de/kontakt/gutschein/

    Now to my question.
    How can I send the Site Title and a Custom field from the site where the user come from to my “voucher order form” ?
