How do I display form on a page?

I think I must be missing something. I can create and edit a form, but I have no clue or see no button to assign the form to a page or post.

I am able to insert it as a widget in the sidebar however this is not how I want to use it. I need to insert it into either a page or post.

    1. Am I missing something such as a file?
    2. Is the a special code to use to insert the form into a page or post?

Please CC my web-guy with instructions. admin@texasmatrix.com

  4. Ajax submission is simply a preference. You can use that submit the form without reloading the entire page.
