How do I populate a Multiple Choice with Single Line Text entry?

I would like to populate a Multiple Choice choice with the data from a single line entry, such as the following:

Single Line Text
Name: John

Multiple Choice:
1) John
2) Anonymous
3) Other

As you can see "John" was taken from the above Single Line Text field. How do I do this?
