How to add an image above radio box fields?

    I am building a cake order form and would like a single image above the check boxes showing thumbnails of the cakes. How do I go about inserting an image above the check boxes?

    The form is here. I would like to put an image above the 6" cakes that is the width of the form.

    You should be able to use an HTML field to place your image - would that work for you?

    You could also try placing your image in the "description" area of the field and in the form settings make sure you have the descriptions set accordingly.
