How to add grid/rank to a form

    I am wondering how to insert a grid or rank question into an online form. I have included an example (created in Google forms) of the type of question I am talking about.

    I think this blog post by Kevin may be of help, it's from a couple of versions back but should still help. Basically you would have to setup a number of multiple choice fields and then style then to get the appearance you want.
