How to populate form with URL title?

Hello, searched around but missed something on how to populate one form field with that page URL.

For bookings, for instance, is there a way to get the same form all over the different items, and then use a conditional way to populate the "House" field with the URL title of the page a visitor is at?

    Thanks!

    Edited to ad:

    My form goes with:

    Name
    Email
    Phone
    Check In
    Check Out
House <<< the one I need populated with URL title.

  2. Yes, you can do this using built in features.

    - Edit the field you want to populate with this value
    - Select the Advanced tab
    - In the Default Value select "Embed Post/Page Title" from the "Insert form field" drop down
    - This will add the {embed_post:post_title} merge code as the Default Value

It will look like this: http://grab.by/9mps

You could do this with a Hidden Field that you give a label of House so the user doesn't even see it so they can't edit it.
