    I create the costum post type, and then I want the html code was this:

  2. I'm not 100% sure what you are asking.

    If you want to use Gravity Forms to create a custom post type post you'd have to use this add-on:

    If you want the content of the post to be formatted the way you outlined in that screeshot, you can do this by using the Content Template option when configuring the Post Body field on your form. The content template supports static text, html, shortcodes and form field merge tags. Whatever you put in the content template is what will be used to create the content of the post.
