A client has a 14-page (400-500 questions) form... has anyone created such a monster (something with hundreds of questions)? Can GF even handle this many questions in a multi-page format?

Gravity Forms can handle this, as long as your server has the memory to handle such a large form submission. There is nothing built-in to Gravity Forms that would limit you from doing such a thing, and I would love someone to give it a go.
