Info Not Submitting Correctly in Entries / Users skipping required fields

  1. I am having an issue. Most of the time the forms work perfect. But every now and then I have a submission in which the name, email, address etc just do not appear. These are required fields so they should be getting submitted. They are neither in the email notification or the actual entries in the admin area. Please help! Thanks!

    I'm having the same issue. We have a 5 page form and we just received an entry with a missing page 3. The missing page has one required radio button with two options. The title of the radio button and what we would expect to be one of the two options drop off of the results notification emails and entry page completely, like the radio button doesn't even exist.

    I can't replicate it. I submitted several tests and all of them have the radio button regardless of selection.

    If I disable Javascript the form does not display at all. With Javascript on, there are no JS errors in the console.

    I tried going to page 4 and editing the input tag's "checked" property by removing it in the Inspector. I also tried deleting both of the radio button nodes. The form submits but the default radio title and option does show in the resulting notification emails and entry pages. So I still do not know why it disappears sometimes.

    I'm using the latest versions of GF and WP and I'm using W3 Total Cache with Object caching on and Database caching off.

    I will look up the IP address in the logs and see what browser they were using. However, it sounds like the Chrome bug clears all of the radio buttons. If that's true, then I doubt it would be the case here, because on page 4 we have a whole page of required radio buttons which all submitted fine on the entry that had missing data.
